Type 2 diabetes is a condition where your body doesn’t use insulin properly or doesn’t make enough of it. Insulin is a hormone that helps move sugar from your blood into your cells for energy. When your body can’t use insulin the right way, sugar builds up in the blood, which can cause health problems over time. Type 2 diabetes is most common in adults but can happen at any age, especially if you have risk factors like family history, being overweight, or not getting enough exercise.
The good news is that Type 2 diabetes can often be managed with healthy habits like eating well, being active, and losing weight. Some people may also need medication or insulin to help keep their blood sugar levels in check. With the right support, people with Type 2 diabetes can feel great and live a healthy life.

Your 45-60 minute appointment includes:

With a care plan: $90 out of pocket
Without a care plan: $150 out of pocket
*If you are not using continuous glucose monitoring or would like to try a different brand a free sensor may be included.
Subsequent Appointments
Your 30-minute appointment includes:
